Thank you for your interest in serving as a volunteer at Cottonwood Creek. We value your willingness to be a part of our team!
There are 2 steps to this process: our doctrinal beliefs section as well as the actual background check. For the beliefs section, please type in your name, answer each question and hit the “submit” button at the bottom of the page. Once you’ve submitted the beliefs statement you will then be directed to the Volunteer Application Form.
On the Volunteer Application form, please fill out the first page as completely as possible to help ensure an accurate match with state/national records. When completed, please click on the “Next” button which will take to you a page where you will have the opportunity to identify ministry for which you are intending to volunteer. (ex: Children’s/Sports/AWANAs/etc…) And finally, click on the “Next” button again to access the final page, which is a consent form to run the background check. The entire process should take between 5-10 minutes to complete.
Must be 18 or older to complete a background check.
